Preparing For Your Session

Welcome to Fortitude Wellness Collective! Whether it's your first encounter with us or with any mental health or wellness provider, we strive to foster a calm, friendly, and professional environment where you'll anticipate each appointment.

Before your first session, we kindly ask you to complete the new client forms at least 24 hours in advance. These forms will be sent via your Client Portal once we confirm your appointment. We eagerly anticipate your visit!

Cancellation or Rescheduling

Please notify your provider at least 48 hours from the time of your scheduled appointment date and time (e.g., by email, phone call, or voicemail) to cancel or reschedule. Otherwise, a late cancellation fee will be charged. You can also reschedule through your Client Portal.

In the event we need to cancel an appointment, we will promptly notify you via email. We'll also offer the opportunity to reschedule at your earliest convenience. Rest assured, this cancellation incurs no cost or penalty to you.
